The need for population-based care
Kaiser Permanente has 8.1 million members in eight geographically distinct regions. We care for our entire population by identifying members in three risk categories—at risk of being overweight, overweight, and obese—and making appropriate weight management strategies available for each group.
Population-based care tailors interventions to the level of risk:
- Primary prevention through counseling and education
- Secondary prevention—early detection and treatment—through weight management programs
- Tertiary prevention—treatment of established disease—through more intensive interventions such as pharmacotherapy, very low calorie diets, or bariatric surgery.:
Program elements—in place or under development—essential to population-based care include:
- Information systems that include necessary data, such as BMI, risk factors, and physical activity levels
- Evidence-based care
- Tools like patient care registries, risk stratification criteria, and outreach/inreach mechanisms
- Cultural competency
- Rapid cycle evaluation of weight management interventions
- Weight Management Programs
