Care Transitions

KP members experience millions of care transitions per year such as hospital to home transitions. These transitions drive member satisfaction and clinical outcomes. This initiative focuses on ensuring KP members experience safe, reliable, high-quality transitions between all settings. Achieving excellence in care transitions is critical to helping preserve superior care quality across KP.

Integrated Cardiovascular Disease (CVD)

The Integrated CVD prevention program focuses on delivering efficient, effective, high-quality, and low cost care for members with or at risk for CVD. Components of this program include weight management, depression screening, tobacco cessation, and primary prevention and reduction of global cardiovascular risk.

Complete Care for Complex Conditions (4C)

The goal for the 4C initiative is to improve health care delivery and costs for our sickest members who use the most health care resources. Promising programs exist in KP, but are not widely implemented. Our current programs are addressing only a fraction of eligible members and there is significant performance variation in the management of resource intensive members. Seventy percent of these members have one or more of five major chronic conditions for which we have expertise and care management programs (coronary artery disease, depression, diabetes, heart failure, and asthma).

Palliative Care

The Palliative Care initiative seeks to provide evidence based human-centered interdisciplinary palliative care to patients and their families in and across all care settings. All KP patients with advanced illness should have the opportunity to understand and make values-based treatment decisions that meet their medical, social, emotional, and spiritual needs.
